How the form works
When you select submit, this page sends what you entered to a Google Form that KM360 controls. Your entry is stored in a Google spreadsheet that only a small number of KM360 team members can open, and Google acts as our service provider for this purpose. If your submission doesn't go through, the page will tell you and offer an email link instead.
How we use it
To respond to you, to tell you about the Aging Data Collective and related KM360 work, and to understand and improve how this page is used. We keep your information only as long as we need it for those purposes.
Who we share it with
We do not sell your personal information, and we do not share it for targeted advertising. We use Google Analytics as a service provider to measure page use; we have turned off Google's advertising features for this page. We may also share information with providers who help us run our business (such as email, form and spreadsheet tools, and hosting), or when the law requires it.
Your choices
- You can block or delete cookies in your browser settings. We also turn analytics off automatically if your browser sends a Global Privacy Control signal.
- Depending on your state, you may have the right to know what we hold about you, and to ask us to access, correct, or delete it. To make a request, email us at the address below.
